On Thu, Oct 19, 2023, Zimbabwe introduced the lenacapavir HIV prevention drug, marking it as one of the first countries to roll out this biannual injection. This new drug aims to address the significant impact of HIV in the country, which has experienced tens of thousands of deaths over the past two decades.
Clinical studies have shown that lenacapavir provides near-total protection against HIV, offering a potential turning point for high-risk groups. However, challenges such as funding constraints, infrastructure gaps, and maintaining patient engagement need to be addressed to maximize the drug's impact.
The introduction of lenacapavir is supported by the United States President’s Emergency Plan for AIDS Relief (PEPFAR) in collaboration with the Global Fund. The drug is provided at no cost to high-risk individuals, including sex workers, adolescent girls, young women, gay men, and pregnant and breastfeeding women.
Despite the availability of daily oral PrEP, adherence has been a challenge due to stigma and unpredictable schedules. Lenacapavir offers an alternative by requiring administration only twice a year.

Dr. Ernest Chikwati, Zimbabwe programme director at AIDS Healthcare Foundation, describes lenacapavir as a "game-changer" but notes that it is not a standalone solution.
The introduction of lenacapavir represents a strategic enhancement in HIV prevention efforts, particularly for individuals facing barriers to traditional prophylactic measures.
